Apply In Person

1. To Test Drinking Water Parameters in laboratory, the applicant has to visit the Ethiopian Public Health Institute.

3. Applicant has to get the application form from the office to test drinking water.
4. Now, complete the application form with appropriate information under relevant sections. Attest it with your signature once completed.
5. Make sure that you have all the documents listed in the “Required Documents” section of this page.
6. Once completed filling the application form, submit it in person to the relevant authority at the office with the documents and samples.
7. Pay the necessary application fees as per the authority request.
8. After submission, the application and other documents will be forwarded for examination and verification processes.
9. And the sample is sent for testing in the laboratory.
10. Once the verification and testing is done successfully, the applicant will receive a formal notification from the office and the applicant can collect it by any applicable means.
