2. Applicant must provide all necessary information in the application form.
3. Documents that are needed for this procedure are mentioned under “Required Documents” section of this page.
4. Applicant should carry photocopies and originals of all the documents while applying for this procedure.
5. Applicant should attach photocopies of documents (whichever is necessary) along with the application form while submitting the application form.
6. Fee for this procedure has to be paid as per authority’s request.
7. If all above-mentioned procedures were followed properly, the license will be issued within 2 weeks by visiting the office.

This licence is necessary for gaining a practical experience in medical and dental field by graduates when they serve in hospitals under the supervision of experienced doctors.

• Documenting and updating patient’s notes
• Writing Accurate and Informative case summaries
