Apply In-Person:

1. Application for approval of veterinary laboratory is available in the Food, Medicine and Health Care Administration and Control Authority of Ethiopia contact link
2. Applicant must enter all necessary information requested in the application form.
3. Documents that are needed for completing application procedure are listed under “Required Documents” section of this page.
4. Applicant must carry both photocopies and originals of all those documents while applying for this procedure.
5. Attach photocopies of documents along with the application form (whichever is necessary).
6. Fee for this procedure is 10,000 Birr.
7. If all necessary information is provided along with the needed documents, applicant can get their approval within 2 days by visiting the office.

• Academic and qualification certificate
• Registration certificate of the veterinary laboratory
• Registration proof from any professional association of veterinary surgeon.

Any Institution or Individuals with certain experience as a veterinary surgeon are eligible.

Fee for this procedure is 10,000 Birr.

This permit is valid for 1 year.
