Apply In-Person:

1. Application for Indexing of Animal Health Students in Ethiopia is available in the Food, Medicine and Health Care Administration and Control Authority of Ethiopia contact link
2. Applicant must provide all necessary information in the application form.
3. Documents that are needed for this procedure are mentioned under “Required Documents” section of this page.
4. Applicant should carry photocopies and originals of all the documents while applying for this procedure.
5. Applicant should attach photocopies of documents (whichever is necessary) along with the application form while submitting the application form.
6. This procedure can be done for free of cost.
7. If all above-mentioned procedures were followed properly, the student index number is available within two weeks.
8. Applicant can collect their registration certificate by visiting the office.

The Index Number is valid until the student finish undertaking their training programme and councils licensing examination.
