1. To apply for an ID card, applicants will need to go to a DUI Processing Centre. A link is provided under 'Office Locations & Contacts' section which you can use to search for the nearest one.
2. Complete an application form and submit it together with other required documents.
3. Applicants who are minors must be accompanied by their parents, or by two witnesses preferably family members that holds a DUI.

• Original and certified copy of Birth Certificate (issued not more than a year ago)
• DUI of either parents
• If the applicant may apply on their own if they possess any of these following documents:
  • Minority card
  • Passport
  • Driver's license
  • Personal identity card
  • Voters ID
• Proof of blood type issued by a authorized laboratory
• Proof of profession

Note: Certificates to be presented must not be issued for more than one year at the time of application.
