1. The minor must be present when the application for the card is submitted to the local municipal office.
2. Complete an application and present it together with the applicant's birth certificate. Note: No need to present the certificate if the child or the minor was born and registered in the municipality where the application is being submitted.
3. Parents will be asked to present their DUI. If the DUI was issued in another municipality, applicants will need to present proof of address (example: utility bill).
4. Pay the application fee.
