Apply In-Person:

1. To Get Help for Person with Special Needs, the applicant has to approach the State Information Service. contact link
2. Applicant must obtain the application form from the relevant department.
3. Applicant has to enter all the information in the application form.
4. Applicant must provide all the necessary information in the application form.
5. Documents that are needed for this procedure are mentioned under “Required Documents” section of this page.
6. Applicant should attach photocopies of documents (whichever is necessary) along with the applicant form while submitting it at the office.
7. Service is free for this procedure.
8. Submit the completed forms along with the supporting documents at the respective office for verification and processing.
9. This application will be processed and information will be verified by the authority.
10. If the verification is successful, the notification will be sent to the applicant.
11. After the notified period applicant gets certificate as per applicable delivery norms.
12. If there is no information regarding the application, then, the applicant can visit the office to know status of the application.

• Signature/Thumb impression of the patient countersigned by the Medial Superintendent/CMO/Head of Hospital (with seal)
• Proof of Age
• Recent attested photograph showing the disability affixed here.
• Domicile certificate as per requirement
• Three passport size photographs of the applicant.
• Egypt card.
• Passport size photograph of applicant (please take 5 copies).
• Any other document to claim benefits offered by respective Egypt government as advised by authorities.
