2. The applicant will need to present his/her identification during the process.
3. Submit the form.

Note:

• Consular offices are entitled to refuse to provide a citizen the service he/she requested, if the consular staff doubt the usefulness for the citizen, and if they think the service will harm the future interests of the Applicant, or that of the Consular Office .

• Have made the Declaration of Death.
• Identification of the applicant.

Consular Offices may require the applicant to submit additional documents.

All citizens have the right to request a death certificate from the consular offices of the Dominican Republic.

This service is free.

Processing time may take about 10 minutes to 2 hours, depending on the urgency of the request, the number of previous requests being processed and staff availability.
