1. Fill in the application form.
2. Submit the form along with all other required documents.
3. Pay appropriate fee.

• Dominican identification: passport or identity card and voter.
• Demonstrate the relationship with the child and if it was declared by a parent, an authorization of at least one parent.
• Have made a declaration of birth of the child in the Dominican consular office.

All parents have the right to request consular offices of the Dominican Republic, to be issued a Certificate of Birth Declaration.

Application fee: 20.00

The amount payable is in U.S. dollars (USD), in European areas it will be EUROS or in the currency of the country where the Consular Office is.
