Apply via Agent
Apply In-Person:
- Approach Single desk Portal of Dept. of Industries for Grant or Renewal of Drug Manufacturing License
- Refer to the “Office Locations & Contacts” section in this procedure to see the respective address and contact details.
- DOCUMENTS TO BE SUBMITTED TO THE LICENSING AUTHORITY ,DRUGS CONTROL ADMINISTRATION BY THE APPLICANT FOR GRANT/ RENEWAL OF LICENCES IN FORM-25, 28, 32 AND 36. You can procure the application form from the respective address mentioned above.
- Fill the form and attach required documents with it.
- Please make sure you have all the documents that are listed down the “Required Document” section of this page.
- Submit the application form to the Drug Inspector along with proper fees.
- The Drug Inspector who verifies the details of the application.
- The Drug Inspector forwards the application to Assistant Director with remarks.
- The Assistant Director verifies the details of the online application and forwards the application to Deputy Director for approval.
- The Deputy Director verifies the application and forwards the application to Joint Director with remarks.
- The Joint Director verifies the application and forwards the application to Director for Final Approval.
- The Director scrutinizes the application and approves the list of products to be retained in the Licenses.
- The applicant can then collect the licence from the Single desk Portal of Dept. of Industries.
Apply Online:
- To renew drug license (Retail and Wholesale Pharmacy) in Daman and Diu, the applicant shall use the following link to apply online: Apply online
- Now hit the “Login” option on the top right corner of the page to get next page. If you are a new user, then use the “New Login ID & PWD” option below Login option on the top right corner to get next page. Please follow the on-screen instructions to register and then login.
- In the given page, click on the drop down list for “State” and choose “Daman and Diu” and complete the prompts as appropriate.
- Now click on the “Submit” button to reach the next page.
- Once login user can change the password and update details as per the page option.
- Once the details are entered for user registration, The applicant shall process his /her online application.
- The applicant shall follow the page prompt in completing the personal details, details of a competent person, address details of premises.
- Upload the required documents and completion of payment by uploading the required details.
- Applicant will get acknowledgement for the online application submitted successfully. Please keep it safe for future reference.
- This application will be processed further.
- The applicant gets a notification about the process updates.
- The drug inspector will inspect the premises for verifying their correctness.
- After the inspection is over, a Competent person will be asked to report for an interview.
- Once all the above processes are over, the controller of drugs will issue a drug license.
- Applicant shall download or receive it on hand or through mail his / her certificate as appropriate.
Apply through Centres:
- To Renew a drug License (Retail and Wholesale) in Daman & Diu, the applicant shall visit the nearest CSC/Tehsil Centre. Find contacts to CSC centres in the link provided:Link
- Procure the mandatory application form from the concerned centre and fill it up with the necessary details required for the license.
- Make sure to attach all the "Required Documents" along with the application form before submission.
- Provide authorized signatory and photograph, wherever applicable.
- Make the requisite payment required for the process (if applicable).
- Now, submit the application to the office of the concerned centre and obtain the acknowledgement slip.
- After successful verification, the License will be renewed by the concerned authority.
- Obtain the license from the concerned CSC centre by showing the acknowledgement slip.
Required Documents
- Statutory form – 19 (one for each license).
- S.B.H. Challan of required value for each license (for each form) for the required amount under the Head of Account 0210-Medical and Public Health, 04-Public Health, 104-Fees, Fines etc. / net banking.
- Declaration by the proprietor / Partner / Director / Competent Persons / Regd. Pharmacist
- Affidavit by R.P / C.P on required stamp paper duly notarized.
- Attested copy of Registered Pharmacist certificate (renewal up to date) affixed with latest original photograph and signature of the candidate.
- Original drug licence.
- Copy of list of Directors downloaded from ROC website signed by Company Secretary / Managing Director (In case of company).
- In case of company an Affidavit under Section 34 of Drugs and Cosmetics Act, 1940 on Rs.20/- stamp paper duly notarized along with copy of its board resolution to this effect signed by company Secretary or Managing Director or Majority of the Directors of the company.

Instructions
- If employed must disclose designation, institution name and place of working.
- Should have valid email ID and mobile no.
- Latest colour photo with white background, not older than a week.
- After Agreeing conditions, click Submit button to complete process at your end.
- For Renewal of Registration Certificate Appearance in Council is not necessary. After uploading necessary documents, Pharmacy Council will intimate you by email and SMS, after verification & approval by Registrar of Council. You can then download Renewal Certificate from your given email id
- Council is not liable for losses caused by any unauthorized use of your account. Keep your Account Password secured.

Need for the Document
- A drug license is required to sell/distribute/store medicine. It can be obtained from appropriate government department.
