1. An authorization letter must be submitted to the Consulate who has jurisdiction in the area.
2. Copy must be sent to the Department of Immigration and Passports Department of Immigration. Fax: 296-6544

• Authorization letter, the letter should indicate:
  • Name of or authorizing, identification number and exact address.
  • Name of or children traveling, birth date, destination and purpose of travel.
  • Name and identification details of the persons accompanying him.
  • Or authorizing signature (the signature must be addressed to Consulo).
  • Certification or proof of child's birth.
  • Identity card or passport of the signatory or signatories of the letter.
  • Photograph of the child.

Note: The signature must be made ??before the consular officer.
