Apply In-Person:

1. The applicant shall contact Revenue Accountant (RA) in the sub-divisional office to obtain an application and agreement form. The form can also be downloaded from website of the department.
2. The A&A form is to be submitted in duplicate. Form link: Link
3. The application and agreement form duly filled shall be submitted to Consumer Clerk (CC)/ (RA) for further processing.
4. RA shall give details of amount to be deposited by the applicant with the cashier.
5. Registration No. shall be allotted by the cashier at the time of receipt of application processing charges. Application No. shall be allotted after deposit of security amount and agreement finalization.
6. The applicant shall keep the original receipt(s) with him /her and leave the case file with the cashier.
7. Authorities will verify the submitted documents; if all the documents are in place then authorities will advise the customer to pay the fee for this procedure.
8. Once the payment is made, a receipt will be issued to the applicant. Keep it safe for future reference.
9. This application will be subjected for internal processing and a technician will inspect the premises on a scheduled date at the presence of applicant.
10. On satisfactory inspection in all respect by the officials payment has to be done accordingly.
11. After payment, connection will be effected according to consumer’s application.
