Under this scheme, financial assistance is provided to severe and multi-disabled persons who are living below poverty line.
Apply In-Person:
For both state and central government pension scheme

1. To apply, the eligible person has to submit two copies of duly filled application form along with required documents to the concerned sampark centre.

3. Obtain the application form from the respective counter OR write on a plain paper OR download using the following link. link
4. The authority at the centre will process the application and return applicant with receipt.
5. This application will be processed by the Social Welfare Department of Chandigarh.
6. They will issue disability Pension card and number.
7. Pension will be issued quarterly through the Sampark centre created by the Social Welfare department.
8. Applicant has to visit the centre to avail the cash payment.
9. In case of rejection of application, the applicant is duly informed.

Apply through Centres:

1. To apply through centres, the applicant has to visit the respective “CSC” centre. Please tap on the following link to locate the centre according to the respective district: link.
2. Make sure to have the documents that are listed under the “Required Documents” section of this page.
3. It is advised to take the documents in original to the kiosk for scanning purposes.
4. The operator will process the application online.
5. On successful submission, the applicant will receive an application number as acknowledgement.
6. The status, viz. Approval or Rejection if any, will be conveyed to the applicant through an SMS.
7. If verified, the applicant will be eligible for the pension.
