This procedure explains various ways to Apply for Cancellation of ACR ICard in Central Luzon.

1. Go to the Office of Bureau of Immigration.
2. Submit the complete documentary requirements.
3. Get an Order of Payment Slip (OPS).
4. Pay the required fees.
5. Submit copy of Official Receipt.

• 2 copies of Letter of cancellation stating reasons thereof
• original ACR I-Card
• 2 photocopies of ACR I-Card (fron and back portions)
• photocopy of the Official receipt of payment for applicable fees
