The first step to apply for disability benefits through Veterans Affairs Canada - is to fill out the proper application form. You can get one through any of the following:

• Printing an Application Package
• Using My VAC Account. If you are registered for a My VAC Account, you can submit your application for disability benefits through this site. If you aren't registered, you can register now.
• By Visiting one of the Area Offices or a Service Canada Centre
• Calling Veterans Affairs Canada
• Contacting a Service Officer

Service officers with The Royal Canadian Legion or the War Amps of Canada can also assist you with every aspect of your application, including helping you get all of the information you need to support your application such as service records and medical documents. Their assistance is free of charge.

Your application must contain information that provides details of your disability and your service. Please make sure you have all the information listed below before submitting your application. This will help us review your application and get a decision to you as soon as possible. If we don't have all of this information, then the decision-making process will take longer.

Once you have completed your application package, you can mail or hand deliver your application package to one of our area offices or a Service Canada centre. To find the office nearest you, view our map.

If you use My VAC Account, your application will be submitted electronically.

• A completed and signed Application Form.
• Your signed Consent for Veterans Affairs Canada to Collect Personal Information from Third Parties.
• A signed Quality of Life Questionnaire for each distinct condition for which you are applying for disability benefits.
• Medical information to support your application (including a current diagnosis of your condition):
  • If you are still serving, we will get this information from your service health records. If the information we need is not in your records, we will contact you.
  • If you are released, you must have your doctor or health care professional complete and sign the relevant medical questionnaire(s).
• Service Information If you are currently serving, include a copy of your Member Personnel Record Rsum (MPRR).
• If this is your first application to Veterans Affairs Canada, we will also need a "Proof of Identity" such as a copy of your driver's license.

As a Veteran or other qualified individual, you may be eligible for the Health Care Benefits (Treatment Benefits) program. This program provides financial support for health-care services or benefits available through Veterans Affairs Canadas (VAC) 14 Programs of Choice (POC). Expenses you incur when travelling to receive treatment services or benefits may also be reimbursed.

You may qualify to receive financial support for treatment benefits, if you are:

• in receipt of a disability benefit;
• in receipt of services through the Veterans Independence Program;
• in receipt of financial assistance through the Long Term Care program; or
• in receipt of the War Veterans Allowance.

• Your full names as they will appear on the card. Only you are authorized to use your card.
• Client Number: This is your unique identifier. Please quote this number whenever you call or write us.
• Program: Each number (1 through 14) represents a medical benefit or service. For more information about each of these, see Programs of Choice.
• Group: Identifies your coverage for each of the Programs of Choice.

The VAC Health Care Identification Card provides an efficient and effective method of identifying the benefits that clients are eligible to receive.

Your VAC Health Identification Card identifies which medical benefits or services we cover for you.

You must present your card, along with the necessary prescription, to a provider of your choice who is a participant in the program.

If you do not know any participating providers, you can call the client information number indicated on your VAC health identification card.

Your VAC Health Identification card can be used in all provinces and territories. However, before you travel outside your home province, you should contact the Veterans Affairs Canada for advice on how to obtain your services outside your home province. If a provider in another province will not accept your card as payment, you may have to pay for the benefit and then request reimbursement.

To obtain benefits or services, usually, all you will need to do is to present your VAC health card to a health-care services provider. To ensure VAC coverage, the provider may also need a prescription from your doctor or medical professional and, if required, your pre-authorization from VAC.
