Personal InformationEdit

Name ChangesEdit

All documentation, including your passport, should be issued under the same name. If you have changed your name on other identification documents, your passport must be in this same name.
For any name change, you must apply for a new passport:

• Submit a complete adult general passport application (you cannot use the passport renewal process for a name change), that includes:
  • the application form completed in full;
  • proof of Canadian citizenship;
  • supporting identification in your new name;
  • two photos, one of which is signed by the guarantor;
  • the required fees; and
  • any valid Canadian travel document, if applicable.

Plus
For a married surname change, you may be asked to provide your:

• marriage certificate (original or copy).
• For a common-law name change, include a signed letter stating that you are in a common-law relationship and that the co-habitation has been ongoing for more than one year.
• For any other legal name change, include one of the following:
  • a legal change of name document;
  • an adoption order; or
  • a birth certificate or citizenship certificate issued in the new name.

Place of BirthEdit

Your place of birth must be provided on the application form.
You may request that your place of birth does not appear in the passport. However, if you chose to do so, note that:

• Place of birth is mandatory for entry to some countries. You should contact the consulate or embassy of every country you plan to visit to ensure that you will be allowed entry if your passport does not indicate your place of birth.
• You may have difficulty obtaining a visa.
• You may experience delays at border crossings.

It is your responsibility to verify requirements before traveling.
If you do not want your place of birth to appear in the passport, you must complete and submit with your application a "Request for a Canadian passport without place of birth (http://www.pptc.gc.ca/form/pdfs/pptc077-eng.pdf).

SignatureEdit

Your signature must not exceed the inner limits of the signature box on page 1 (Declaration of Applicant) and must appear on all three pages.

DutiesEdit

1. Complete and sign the "Declaration of Guarantor" section of your application form.
2. Write, "I certify this to be a true likeness of (name of applicant or child)" on the back of one of the passport photos and sign.
3. If applicable, sign and date a copy of each document to support your identity.

Note: The applicant must not assist the guarantor in performing his or her duties. If your guarantor requires assistance, contact Passport Canada.

Eligible GuarantorsEdit

Your guarantor must:

• Hold a five-year Canadian passport that is valid or has been expired for no more than one year, on the day you submit your application;
• Have known you (the applicant) personally for at least two (2) years.
• In the case of a child, the guarantor must have known you (the parent or legal guardian who is applying on behalf of the child) personally for at least two (2) years and must have knowledge of the child;
• Be a Canadian citizen 18 years of age or older;
• Have been 16 years of age or older when he or she applied for his or her own passport;
• Provide the requested information contained in his or her passport;
• Be accessible to Passport Canada for verification;
• Live in Canada, in the United States or in any of the other areas where submission of the "in Canada and in the USA" application form is authorized (Bermuda, American Samoa, Guam, the Marshall Islands, the Midway Islands, the Northern Mariana Islands, Puerto Rico and the US Virgin Islands). It is not necessary that the guarantor reside in the same country as the applicant.

Family MembersEdit

A family member or any individual residing at your address may be your guarantor provided he or she meets the specified requirements. If you are the parent or legal guardian applying for a passport for your child, you cannot act as guarantor; however, the other parent can.

Declaration in lieu of guarantorEdit

If you have not known an eligible guarantor for at least two years, complete form PPTC 132 "Statutory Declaration in Lieu of Guarantor" available from any Passport Canada office or by telephone. Important: A person listed as a reference on the Statutory Declaration in Lieu of Guarantor cannot be listed as a reference on the passport application. The "Statutory Declaration in Lieu of Guarantor" form must be sworn to or declared before, and signed by, a person authorized by law to administer an oath or a solemn declaration. If completed outside Canada, a qualified official includes a Canadian or British diplomatic or consular representative, or a qualified local official.
Incorporated Institution (in Quebec) may also sign as guarantor until the adoption is final (probationary adoption).

The application may be submitted by a third party provided they:

• show identification,
• are 16 years of age or older,
• are either your:
  • parent,
  • child,
  • parent's spouse or common-law partner,
  • child's spouse or common-law partner,
  • spouse or common-law partner,

• You may submit original documents. They will be returned to you.
• If you are submitting photocopies, you must copy both sides of the document and have your guarantor sign and date the photocopies (both sides) to indicate the originals have been seen. If you are submitting photocopies (both sides) but are using form PPTC 132, "Statutory Declaration in Lieu of Guarantor" (available from any Passport Canada office or by telephone), or are using form PPTC 326, "Statutory Declaration in Lieu of Guarantor for a Travel Document", the official who signs the "Declaration" must also sign and date the photocopies of your identification to indicate the originals have been seen.
• An official translation in English or French must be provided with any documents that are written in another language.
• If you have no documents to support your identity:
  • If you are living in Canada, contact Passport Canada.
  • Otherwise, contact the nearest Government of Canada office.
• Failure to provide the proper documentation will result in the application being rejected.
