Note:If you submit your application in person, you should do so yourself. If this is not possible, it may be submitted in person by an acceptable third party. You must be 16 years of age or older to submit an application in person. Acceptable Third Party
The application may be submitted by a third party provided they:

• show identification,
• are 16 years of age or older,
• are either your:
  • parent,
  • child,
  • parent's spouse or common-law partner,
  • child's spouse or common-law partner,
  • spouse or common-law partner,
  • spouse or common-law partner's:
  • parent,
  • parent's spouse or common-law partner,
  • child, or
  • child's spouse or common-law partner.

The people in the list above may or may not reside at the same address as you. In all cases, the person must provide identification. If the identification does not prove that the person lives with you, he or she must also bring a written statement signed by you, authorizing him or her to submit your passport application.

Any relative residing at your address can also submit an application on your behalf provided that they:

• are 16 years of age or older, and
• show identification confirming their place of residence. If the identification shown does not contain an address, further documentation to establish that the person resides at the same address as you will be required.

Passport delivery or pick-upEdit

Once its ready, the passport (and any original documentation you submitted) will be returned to you by traceable mail - unless you applied in person at a Passport Canada office and made pick-up arrangements when you applied (extra fees apply). A third party may pick up the passport in person on behalf of the applicant if written authorization from the applicant is provided.

Once you receive the child's passportEdit

Make sure the information in the passport is accurate, especially the childs personal information. If there are any errors, contact Passport Canada immediately to have the problem corrected.

Signature on a child's passportEdit

• Parents must never sign the passport of a child.
• Children 11 years of age or over must sign the passport, in ink, in the signature block on page 3 only if their signature appears on page 2 of the passport. Otherwise, the signature block on page 3 should be left blank.
• Children under 11 years of age do not sign the passportthe signature block on page 3 must be left blank.

Address and emergency contact informationEdit

• Complete the section provided on page 4. For the emergency contact, we recommend that you name someone who would not normally travel with the child.
• If the address or emergency contact changes, cross out what is there and write the new information in the space above. Do not use correction fluid. If there isnt enough space on page 4, write the new information on a separate piece of paper and insert it into the passport.

Children in Foster CareEdit

(Or in the process of adoption in Canada)
Depending on the applicants relationship with the child, the child passport application form must be accompanied by one of the following:

• Form PPTC 463A when the applicant is the director of Family Services, social worker or child protection worker (http://www.pptc.gc.ca/form/pdfs/pptc463A-eng.pdf).
• Form PPTC 463B when the applicant is the foster parent or the parent (http://www.pptc.gc.ca/form/pdfs/pptc463B-eng.pdf).
• Form PPTC 463C when the applicant is the licensee or representative of a licensed adoption agency (http://www.pptc.gc.ca/form/pdfs/pptc463C-eng.pdf).

Forms PPTC 463A, PPTC 463B and PPTC 463C specify what documents must be included with the application, as well as the required information, depending on the applicants relationship with the child. These forms also specify who qualifies as a guarantor, depending on who is applying, and the information the guarantor must provide.

The forms PPTC 463A, PPTC 463B or PPTC 463C must be filled out completely (no sections left blank) by the appropriate person and must always be accompanied by a Child general passport application form (PPTC 155, PDF, 423 KB).

When submitted with forms PPTC 463A, PPTC 463B or PPTC 463C, only complete the following sections of the Child general passport application form (PPTC 155, PDF, 423 KB):

• Section 1Child's Personal Information;
• Section 2Information on Applicant
• the first two questions; and
• the information on the Applicant (parent or legal guardian) and Other parent or legal guardianto be completed by the childs parent or legal guardian (other than the director of Family Services or the adoptive parent(s)), if the parent/legal guardian retains custody or parental authority or shares custody with the adoptive parents;
• Section 3Previous Canadian Travel Document
• Section 4Proof of Canadian Citizenship; and
• Section FCredit Card Information, if applicable.

Additional instructions
If the applicant is:

• the parent and is also the legal guardian or has parental authority of the child; or
• the legal guardian of the childother than the director of family services or the adoptive parent(s); then,
• complete only the Child general passport application form (PPTC 155, PDF, 423 KB).

However, if the parent who is applying is not the legal guardian or does not have parental authority of the child, the following documents must be submitted with the completed application form and other required documents:

• a letter from the childs legal guardian or the person who has parental authority authorizing the applicant to apply for the childs passport and authorizing the release of the passport to him or her;
• a copy of the court order or agreements relating to the involvement of Family Services with the child, if such an order or agreements exists. In the event that a copy of the court order or agreements cannot be forwarded to Passport Canada for confidentiality reasons, the passport application must be accompanied by a summary of the order or agreements signed by the director of family services or the agencys lawyer. The summary must indicate:
  • the childs legal guardian or the person who has parental authority;
  • whether a mobility restriction clause exists, and if so, what it entails; and
  • the date and number of the court order, if possible.
• in the case of a probationary adoption, a letter from the director of Family Services confirming that the child is undergoing an adoption process, and the name of the future adoptive parents;
• in the case of a private probationary adoption, the custody or guardianship agreement signed by the childs biological parents and the future adoptive parents for the duration of the adoption process, if such an agreement exists.

Persons Adopted AbroadEdit

• Consult Citizenship and Immigration Canada for information about citizenship and international adoption.
• Persons adopted abroad by Canadian citizens may immediately be granted citizenship provided the adoption meets certain criteria. Individuals adopted abroad and granted Canadian citizenship are entitled to Canadian passport services.

Returning to Canada with an adopted childEdit

• If you are traveling overseas and wish to apply for a Canadian passport to return to Canada with your adopted child, a passport application can be submitted at a Government of Canada office abroad as soon as the adoptee has been granted Canadian citizenship.
• Note: You should submit the passport application at the Government of Canada office that processed the grant of citizenship or the Government of Canada office responsible for issuing passports to Canadian residents in the country where the adoptee resides (if different).

Adopted persons under 16 years of ageEdit

For children under 16 years of age, adoptive parents should come.

List down the documents required for successfully completing the procedure.

• The custodial parent in the case of separation or divorce, or

If you submit the passport application in person: A parent, legal guardian or authorized third party may submit an application in person on behalf of a child. Passport Canada may also contact any other parent or legal guardian.
