To replace your drivers license, go to any driver licensing office with:

• Legal proof of your name-change such as
  • a marriage certificate issued by Vital Statistics, a religious organization or a marriage commissioner
  • a divorce certificate (both the married and previous names must be on the document).
• Required ID. These can be in your new or old name.
• Fee to pay for your new license.

Important: All identification must be original documents. Passports are not accepted.

A photocopy is accepted only when it is certified (stamped) as a true copy of the original document by the government agency that issued it.
