How to Apply
If you have lost your Aircrew licence or it has been damaged in some way and you require a replacement with the relevant authority; please follow the following steps.

Apply In-Person

1. You should start by visiting a police station to report the loss/damage of the Aircrew Licence. You are also to acquire a police abstract/report from the police authorities which is a prerequisite to making the application.
2. You are also to swear an affidavit of the circumstances surrounding the loss, theft, mutilation or damage. You are to attach this affidavit with your application.
3. Applicants should then visit the the CIVIL AVIATION AUTHORITY OF BOTSWANA {CAAB) head office to apply. Please view the CAAB contact details via this link:Link
4. Applicants are required to complete and submit the prescribed APPLICATION FORM FOR DUPLICATE AIRCREW LICENCE, forms are available for free at any of the CAAB offices.
5. Submit the completed form attached to the supporting documents, listed under “required document” section of this page to the registration and licensing receiving office at the CAAB head office.
6. The CAAB agent will verify if all the required documents and information have been provided, and if the application is compliant if everything is in order, the application file will be registered, upon paying the prescribed fee, as mentioned under the “FEE” section of this page.
7. Applicants will then be issued with an acknowledgment letter, containing details of the application.
8. The application file will be assessed and processed by the registration and licensing unit, based on the information and documents that have been provided and if complying;
9. The application will be approved and you will be notified when the license is ready for collection by phone or mail.
10. Proceed to the CAAB registration and license office and present the acknowledgment letter and identity document to be validated by the licensing office and have your photo taken, after the photo is taken, sign on the signature pad/book and collect your printed Duplicate Aircrew license at the dispatch office against your identity document.

• Four passport size photographs - full face
• An original Identification Card or Passport and a copy of the same
• Police report/ abstract
• Payment receipt of the prescribed fees
• Any other additional documents requested for be the CAAB
