Apply In-Person:

1. One should conduct and Visit Mineral Affairs Division (MAD) at MMEWR Headquarters for consultation and file the application with the department
2. Application form, (Form I) can be obtain from the above offices, or click the below link to download Portal
3. Accurately fill in the form and attach all the requirements and submit them for processing and verification
4. Pay the processing fees and Book with MAD the date and time for diamonds to be inspected by the department.
5. If the MAD officer is satisfied that you have complied with the all conditions as set for the application of the certificate.
6. The certificates will be issued by the Diamond Office under Mineral Affairs Division (MAD), within the Ministry of Minerals Energy and Water Resources.

• An invoice and a packing list showing value, carats, and legitimacy of the rough diamonds is needed fr both exports and imports of diamonds.
• Any other document that may be specified by the authorized officer
