Birth of a child outside of a health institution should be reported by the childs father if the father is unknown or cannot register the birth. The birth should be registered by a member of the family or person present at birth or the mother if shes able to do that.

• The date, name and signature of the certifying Registrar, Acting Registrar or Secretary, and the office seal.

Every child has the right to be registered and birth of a child must be registered within 15 days. Birth of a child in a health institution is reported by the health institution to the competent authorities
