You will first need a Memorial prepared by a lawyer requesting the certificate. Then you will need a range of supporting documentation including full confirmation of the property details, and copies of the Carnets of two witnesses who can verify that you live there.
When you submit your application the office will make an appointment for you to come to the police station and then accompany a police officer back to your residence to prove that you live there.
Once they have confirmed your address, they will then prepare the certificates which will be available to collect the following day.

You can get this from the police station (called PTJ) where you will have to acquire a certificate stating you live where you say you live in Bolivia.

To apply for a long-term visa, you must register a fixed domicile and request a certificate that proves you have no criminal record in Bolivia.
There are certain documents which will need to be signed by a Bolivian neighbour stating that you are living where you claim to be living, and by a lawyer (another 50 Bolivianos). Both of these people will have to give you photocopies of their ID cards.
You can only fill these in once you have the legal contract showing you are renting the property you say you are renting. Youll also need colour photocopies of your passport and you landlords ID card. Not to mention proof that your landlord has paid tax and their utility bills.
