Required Documents

  • Birth certificate,
  • Bolivian ID
  • Parents applying on behalf of their minor children may use their own passport or other government issued identification with the bearers name, photo, and signature.
  • Parent(s)/Guardian(s) must submit evidence of their relationship to the minor applicant.
  • One recent photographs measuring 5cm by 5cm, taken full face with a light background.

When seeking a passport for a minor under age 16, in addition to document required both parents must come with the child and their own passports or other government-issued identification with the bearers name, photo and signature. If both parents cannot come, the absent parent must provide a notarized written authorization form (Statement of Consent or Special Circumstances: Issuance of a Passport to a Minor Under Age 16) for the issuance of the passport and include the absent parents passport or a notarized copy of the absent parents passport/identification.
