Apply In-Person:

1. Visit the registry general during working hours to obtain the application form
2. Fill in the application and attach the required documents to it.
3. Chose the category of death certificate you wish to apply for. E.g. express or Normal process
4. Submit your application form for verification and pay the appropriate fee
5. The registry general will then process your application and produce the death certificate

Apply through E-mail:

1. If you don’t reside in Bermuda, you can request for the Death Certificate by obtaining the certificate order form through the government of Bermuda website: website or download it direct using this link:form
2. Complete the form with correct information
3. Pay for the application fee. Note that you must pay by credit / debit card if you use a Certificate Order Form.
4. Attach the required documents to the form and send it to the register general through their email address:taarchibald@gov or bmorrgdeaths@gov.bm
5. Upon receiving your application the register general will process your application and send it to you via the address you provided

• If you apply for the Death certificate using certificate order form, you must pay the application fees via a pay by credit / debit card and indicate the card information of the application form

• Name
• Certificate registration number
• Payment related information

• This document proved that death has occurred.
