Apply Online:

1. Visit the Bermuda Government Website: Apply Online
2. Underneath the information bar in the caution maroon bar, select COVID 19 Forms
3. Scroll down to “Traveller Screening From”.
4. Fill in the online application form, agree to the declaration then submit.
   
   

NOTE: all persons arriving in Bermuda will be subject to mandatory quarantine in a Government approved facility. If you are not willing to remain in a government quarantine facility for 14 days, you will not be landed in Bermuda.
