Apply In-Person:

1. Download and complete the application form in “Documents to use” section below.
2. Attach the required document to the application form and visit the Department of social insurance office to submit your application.
3. The official will verify your application and forward it to the back office for processing
4. You application will be processed with 3 to 5 working days and you will receive your social insurance number via mail.

1. Alternatively, Obtain and fill the application form “Documents to use” section below
2. Attach the required documents to the application form and send it to the Department of social insurance office by their mailing address;

• A copy of your birth certificate or passport, if born in Bermuda
• A copy of your birth certificate or passport and your immigration spousal letter, if married to a Bermudian
• A copy of your birth certificate or passport and your work permit, if a non-Bermudian
