1. Complete 'Form 6 - Declaration of Lost, Stolen or Mutilated Belize Passport' (a link is provided under 'Documents to Use' which you can use to download the form).
2. Sign the form in the presence of the Commission of Affidavits, Justice of Peace or a Notary Public.
3. Submit the Completed Application FORM 6 along with one passport sized photograph, completed passport application form and your supporting documents.
4. Pay the application fee.
