Apply In-Person

1. The Applicant has to submit an application in Form IV to the Hospital and Healthcare Facilities Board, The Bahamas. Link to Contacts
2. The Applicant can download the Form from the following Form Link.
3. Complete the application form with all details.
4. After filling in all the details, submit the Application Form to the Hospital and Healthcare Facilities along with the corresponding fee.
5.  The application must have the following requirements submitted to the Facilities Board:
6. Satisfy the Board that the applicant is a fit and proper person.
7. Payment of the prescribed fee.
8. Satisfy the Board that the hospital or healthcare facility would be operated in the public health interest.
9. After approval, the Board will issue the license.
10. The applicant must collect the license and display it in a prominent spot at the facility.

List down the documents required for successfully completing the procedure.

1. Proof of child birth from the Hospital.

Any person possessing a valid Hospital and Healthcare Facilities license can apply for renewal.
