Apply In-Person

1. To Obtain License for Medical Practitioner, the applicant must submit completed form with supporting documents as mentioned in the Required Documents section of the procedure along with corresponding fee to the Health Professions Council.
2. The applicant can download the form from the below link: Form Link.
3. All non-Bahamians must submit a current work permit or contract along with their application. (Only a notarized copy will be accepted if the original cannot be presented for verification).
4.  All persons registered are eligible for a licence that confirms them as a fully licensed medical practitioner, upon payment of $200 fee.

Persons must be registered medical practitioners.

• BS$210.00 ($200.00 application fee, and $10 licence identification fee).
• BS$35 (replacement fee if lost).
  

The License is valid for one year and should be renewed.

It is required to obtain has a medical practitioner.

If your license is lost, damaged or destroyed, it can be replaced with a copy issued by the Council. A $35.00 application fee payment is required.
