1. The Person has to visit Adolescent Health Centre on Physical Examination Clinic days (Tuesdays & Thursdays 8:30am - 11:00am).

3. The Person has to Register at the front desk. Wait until your name is called in the waiting room.
4. The Person has to take part in the Medical examination.
5. Follow the suggestions that have been given to you.

• Picture ID (both client and parent/guardian for first visit)
• Parent/guardian written consent (clients under 18 years)
• Accompanied by parent or guardian for first visit (clients under 18 years)
