Apply through Meeseva

1. Application to change your date of birth in the birth certificate can be collected from Meeseva centre.

3. Applicant can also download the application form from the following link:Application Link
4. Fill the application form and attach all the required documents
5. Go to your Meeseva centre. Submit the completed application form, enclosing entire required document as per our “required document” section and submit it along with the fees as applicable. Meeseva operator will do the online process.
6. The applicant will receive an application number as acknowledgement. The applicant will also get transaction number as text message on his registered mobile number.
7. The online details will be processed by respective department.
8. Applicant can fill the application number and transaction id to know the current status of his/her application in the following link : Link for status
9. The important factor to check the record from the Medical Institution, who conducted delivery will be verified.
10. If the record is available, and it has the correct date as required by applicant to change in birth certificate then the work is easy.
11. If the date in the Medical Institution record is the same as in the birth certificate, then the applicant will be informed to get the letter from the Medical Institution, who conducted delivery supporting the requirement as guided by authorities.
12. On decision of the respective department authority, applicant will get his or her Certificate from Meeseva centre by producing the acknowledgement.

Apply In Person

1. To Change Date of Birth in Birth Certificate, the applicant has to approach the office from where the birth certificate was obtained.
2. Please visit the respective office – Urban Local Bodies link Commissioner and Director of Municipal Administration link
3. Obtain 'Birth Certificate Update/correction Form' from the respective counter or write in an A4 sheet as advised to apply.
4. check with the department authorities that the record from the Medical Institution, who conducted delivery is available.
5. If the record is available, and it has the correct date as required by applicant to change in birth certificate then the work is easy.
6. If the date in the Medical Institution record is the same as in the birth certificate, then the applicant should get the letter from the Medical Institution, who conducted delivery supporting the requirement as guided by authorities.
7. Complete the application form. Affix court stamp as notified by authorities.
8. Attach the necessary documents which is mentioned in the “Required Documents” section of this page.
9. The fee for this procedure has to be paid as per the authority’s request.
10. Submit the all the documents along with the form to the authority officer.
11. Once all the process gets completed, the applicant gets SMS or call from the office.
12. The applicant has to visit the office to get the certificate.
13. The processing time for this procedure is up to 15 – 20 days.
