Apply In Person:

1. To obtain a birth certificate for the animal, the applicant must approach the Veterinary Hospital of their respective area. The address and contact details of all the Veterinary Hospitals ofAndaman & Nicobar Islands can be found in this Contact Link
2. Collect the applicable application form from the office.
3. Fill the application form with the required information.
4. Attach the required documents as per details given in the “Required Documents” section of this page.
5. Submit the application form at the Veterinary Hospital office and make the required payment.
6. After submission, the Applicant has to wait until the application is processed.
7. The process usually takes 1 week.
8. Applicants get notification via SMS.
9. After that, the applicant can collect the birth certificate from the office.

• Must have a Vaccination card (against any diseases) of that particular animal.

• Payment will have to be made as per the requirement of the Office.
• Additional fees may be charged for the process.

• The birth certificate of Animal is valid for a lifetime.

• The Birth Certificate is required to identify the Animal Background and Genetics details.
