Andaman and Nicobar - Apply for State Disability Pension


Procedure

Under this scheme, financial assistance is provided to severe and multi-disabled persons who are living below poverty line.

Apply In-Person:

Assistance to Disabled Persons Scheme (ADIP)

  1. To apply for this scheme, the eligible person has to submit two copies of duly filled application form along with required documents to the concerned Block Development Officer, Gram Panchayat In rural , Municipality/ Sub Collector Office In urban.

In the given portal page, please select “ABOUT DISTRICT> Who’s Who” OR“ABOUT DISTRICT>ADMINISTRATIVE SETUP> Wards and Panchayats” OR “DIRECTORY > PUBLICUTILITIES>Municipalities” as appropriate for the contact details.
  3. Obtain the application form from the respective office OR write on a plain paper Or download using the following link: Form Link
  4. After scrutiny, the application is forwarded to the concerned authority for approval.
  5. Following the approval, pension will be remitted monthly through cash on the15th of every month irrespective of it being a holiday at the concerned G.P. Headquarters or BDO office in the rural areas and at the Municipality / N.A.C. office in urban area or as per state norms.
  6. In case of rejection of application, the applicant is duly informed.

Apply Online:

  1. To apply for a state disability pension online, the applicant has to visit the following website: Link.
  2. Now go to the "Forms" section of the page and download "Pension Form".
  3. Fill up the form with the appropriate information as required and attach all the necessary documents along with a photograph in the application form.
  4. In case of PwD below the age of 18, the applicant must furnish a certificate from the state institute of education.
  5. The applicant is also required to produce a certificate of income through a tehsildar along with an affidavit.
  6. On completion, the applicant shall send the application to the concerned authority.
  7. On issuance of a pension, the applicant will be notified via registered phone number.
  8. The applicant may receive the pension as the registering officer directs.

  • Signature/Thumb impression of the patient countersigned by the Medial Superintendent/CMO/Head of Hospital (with seal)
  • Proof of Age
  • Recent attested photograph showing the disability affixed here.
  • Domicile certificate as per requirement
  • Three passport size photographs of the applicant.
  • Aadhaar card.
  • Bank account details
  • Mobile number.
  • Passport size photograph of applicant (please take 5 copies).
  • Any other document to claim benefits offered by respective state government as advised by authorities.
