Apply by mail

1. To Request for Registration under the Gambling Devices, the applicant has to approach the US Department of Justice link
2. The applicant has to download the form from the following link

   Note: Depending on the browser you are using, select either “Save target as…”, “Save link as”, or “Download” to download the request form to your computer. Run Acrobat Reader from your computer.

3. The applicant has to enter all the details in the application form.
4. Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
5. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
6. Submit all the documents with the completed form to the office mail address.
7. The mailing address mentioned in the “Office Locations & Contacts” section of this page.
8. There is no fee for this procedure.
9. Once submitted, the applicant will receive a return e-mail advising receipt of the request.
10. If the request has been processed, the applicant get the conformation e- mail.

    NOTE: Please save an electronic copy of the request form, as the applicant can use this same form to re-register in the future.

11. Once all the process gets done, the applicant get the registration permit through mail.
12. The processing time for this procedure is up to 4 to 6 weeks.

• Any person or entity engaged in the business of manufacturing, repairing, reconditioning, buying, selling, leasing, using, or making are eligible to make this application.

• There is no fee for this procedure.

• The validity for this registration is one year.
• The request permit expiry in every December 31st.

• A business of manufacturing, repairing, reconditioning, buying, selling, leasing, using, or making available for using the gambling device by others before any such device enters interstate or foreign commerce, needs to register a request in Department of Justice.
