1. The applicant has to reach out to local social security office for the application. Contact details are mentioned in “Office Locations & Contacts” of this page or here is the Contact Link
2. The applicant should make sure to carry all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
3. Applicant has to fill in all the necessary details in the form and attach the photocopies of the documents that are needed along with the application form before submitting.
4. The fee for this procedure has been mentioned under the “Fees” section of this page.
5. Concerned authorities will look into the application form and submitted documents before providing the card.
6. Acknowledgement letter will be provided within 30 days.
7. If the acknowledgement letter has not been given within 30 days, the applicant can visit the office to know the status of their application.

2. If the applicant is an existing user, they can log in directly using their “Username” and “Password”.
   • If the applicant is a new user, they have to register first by clicking on the “Create an account” option.
   • The applicant has to provide their own email address when they create an account.
   • While registering, the applicant has to provide all necessary information in the application form and finally click on the “Create an account” button.
   • Once the registration is successful, the applicant can log in with their “Username” and “Password”.
   • After login, select the “Replacement Documents” tab, and select “Mail my replacement Medicare Card”.
3. The applicant must provide all information necessary for the application procedure online.
4. The fee for this procedure has been mentioned under the “Fees” section of this page.
5. Concerned authorities will look into the application form and submitted documents before providing the card.
6. Card will be provided within 30 days through mail.
7. If the card has not been given within 30 days, the applicant can call 1-800-772-1213 (TTY 1-800-325-0778) to know the status of their application.
