Apply In-Person:

  1. To Renew Permanent Medical Parking (SSA), the applicant has to approach the U.S Social Security Administration link
    NOTE: Enter the Zip Code and click the “Locate” button to check the regional address.
  2. The applicant has to get the form from the respective department or use the following link to download the form link
  3. The Applicant should make sure to carry all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
  4. Applicant has to fill in all the necessary details in the form and attach the photocopies of the documents that are needed along with the application form before submitting.
  5. Once completed, the applicant should submit the Form to U.S Social Security Administration department office or by fax - (410) 597-0455.
  6. Fee for this procedure has to be paid as per the authority’s request.
  7. Concerned authorities will look into the application form and the document.
  8. Once the process is done, the applicant can get the document by visiting the office within one week.
