Apply In-Person
- To apply for a Birth Certificate, you must first register the birth.
- The birth has to be registered with the concerned local authorities within 21 days of its occurrence, by filling up the form prescribed by the registering authority.

- Please use the respective selection to get the contact information for visiting the authorities in ward level to apply in person.
- The information of any birth is given to the related wards on prescribed form within 21 days and then certificate is issued immediately free of cost by the person employed at Registration ward.
- After 21 days till 30 days, the information has to be given as above on prescribed form to the related ward but now Rs2/- is charged as late fee.
- After 30 days and till 1 year, a permission letter from Additional Health officer is to be presented for registration of birth and a late fees of Rs5/- is to be paid. This information on prescribed form is submitted to the related ward according to the birthplace.
- After 1 year delay, the order is to be presented from deputy DM and a late fee of Rs10/- is payable. Now the registration of birth is done at Head office, Health department. Rs. 5/- is paid as the fees to get birth certificate.
- Normally the dispensaries or hospitals where the birth takes place maintain a register and the same is sent to the local bodies at regular intervals for registering the records.
- Once it reaches the Municipality or Revenue Department, the concerned citizen has to approach the authority and apply for the certificate on any working day.

- Once the form along with documents were submitted, Authorities will accept the form and register the birth by verifying the details and documents to issue the certificate.

- Please go to online portal
- In the given page please select “Citizen Login (e-partner)” from the options provided on top of the page to reach next screen.
- Please login using the prompted details. If new user, then click the option “New User Registration?” and follow the screen prompt to register.
- Once login, user gets a page to view the details for the login. Here applicant can use the respective option to change the details if not correct. Otherwise use the appropriate tab to get the next page.
- User shall select the respective choice for certificate from the list of menu option as provided next to Welcome.
- Choose the respective menu option and select Birth certificate from drop down menu to get a option. Click on the option to get next page.
- In this page, user will get a form and should fill / select details as prompted followed by uploading documents (Aadhaar must) and hit applicable tab to reach next page for making online payment.
- In the Payment gateway page use respective mode to pay by filling required details.
- Once the online payment is over, user gets acknowledgement in Mobile as well as in system. Keep it safe for future reference.
- From this point on wards the process will be same as how they processed in apply in person option to issue the certificate.
- Applicant can download the certificate once the intimation on mobile reaches him for approval.
Print Certificate
- Please go to online link
- In the given page please select “Citizen Login (e-partner)” from the options provided on top of the page to reach next screen.
- Please login using the prompted details. Once login, User shall select the respective choice for printing certificate from the list of menu option as provided next to Welcome.
- Please follow the screen prompt to get the printout of the certificate by using the application number generated during registering the details.
Required DocumentsEdit
- If the birth takes place at a dispensary/hospital, the register record is enough.
- Address proof(Voter ID, Driving License etc.) of parents
- Aadhaar Card of Parents
- If the birth takes place at a house, the citizen has to approach the local body and inform in writing. Then it will be verified & the certificate will be issued.
- Application is to be made in the prescribed format of Birth Report Form-1. Link for form form link
- Name of the Parent
- Date of birth of the child
- Location of residence
- Certificate of Doctor / Hospital discharge certificate
- In case of delayed registration, an affidavit from judicial magistrate / Notary is required indicating the date and place of birth, names of parent and evidence of the event of birth
- In case you have applied after a month but within a year you have to obtain a permission from the Add.Dist.Registrar i.e BDO of concerned Taluka
- In case applied after one year, need to obtain a order from executive magistrate and copy of the said order is required to be attached with the prescribed application form.
- If there is no record for birth / death from hospital, then the village head / regional councilor / ma. MP / MBS Certificate of signature and stamp with any one of the doctor should be submitted.
