This procedure explains various ways to Modify or Change Information in Passport in Zamboanga Peninsula.
When you call the DFA Appointment Hotline (02) 737-1000
- All passport applicants scheduled for appearance must appear physically at the DFA at the date and time of their scheduled personal appearance for data and image capturing. This applies to all passport applicants, aging from 0 years old onwards. DFA is strict about the personal appearance system, absolutely no chaperone allowed for passport applicants aging 18 years old onwards.
- Your call will be received by a Customer Care Officer who will assist you in setting an appointment for your passport application.
- After getting your appointment schedule, you will be asked to provide your personal information for the passport application.
- You will be advised on the applicable requirements you need to bring for your passport application.
- You will receive a reference number relative to your appointment schedule.
- Go to passport.com.ph/print to print your application form using the Print Code given by the Customer Care Officer.
- On your appointment date, proceed to the DFA Consular Office located at ASEANA Business Park, Bradco Avenue corner Macapagal Boulevard, Paranaque City (see directory). You must be at the DFA Consular Office 30 minutes before your scheduled time. Early/Late comers will not be entertained.
- Present your printed application form to the Appointment Counter.
- Get your queue number at the Information Counter and sit on the waiting section of the Processing Area. Wait for your queue number to appear on the monitor indicating what processing window you will go to.
- After processing, proceed to the Passport Enrollment Section located at the 2nd Floor. Pay the applicable passport processing fees (see passport fees).
- After payment, proceed to the Encoding Section for data capturing.
- After the Encoding Section, you can have your new passport released and delivered to your home without having to return to the DFA on the release date (optional). If availing of the delivery, proceed to the Delivery Counter to pay for delivery fee.
When you log in to the DFA Appointment Website passport.com.ph
- You will be asked to provide your personal information for the passport application on the online application form. Remember to fill in all the information slots correctly.
- Please take note of the requirements that you need to bring to the DFA Consular Office on your appointment date.
- Choose an appointment DATE and TIME.
- Check your email for your reference number and a link to print your application form. You will need to bring the printed application form to the DFA on your appointment.
- On your appointment date, proceed to the DFA Consular Office located at ASEANA Business Park, Bradco Avenue corner Macapagal Boulevard, Paranaque City (see locator map). You must be at the DFA Consular Office 30 minutes before your scheduled time. Early/Late comers will not be entertained.

Required DocumentsEdit
For married women who would like to use their married surname:
- Marriage Contract (MC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of MC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA.
- Transcribed Marriage Contract from the LCR is required when entries in PSA Marriage Contract are blurred or unreadable.
- Report of Marriage duly authenticated by PSA if married abroad
For spouse of foreign national who would like to use their married surname:
- Marriage Contract (MC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of MC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA.
- Transcribed Marriage Contract from the LCR is required when entries in OSA Marriage Contract are blurred or unreadable.
- Report of Marriage duly authenticated by PSA if married abroad
- Original and photocopy of Commission of Filipino Overseas (CFO) Guidance and Counseling Certificate of Attendance (required for first time applicants or renewal of passport to be used for the first time the surname of the husband)
For married women who would like to revert to their maiden name:
- Birth Certificate (BC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA.
- Transcribed Birth Certificate from the LCR is required when entries in PSA Birth Certificate are blurred or unreadable.
- Report of Birth duly authenticated by PSA if born abroad
- If already widowed: Death Certificate in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) of the deceased husband
- If marriage is annulled: Certified True Copy (CTC) of annotated PSA Marriage Contract and Court Order effecting the annulment
- If divorced: Certified True Copy (CTC) of the Divorce Decree duly authenticated by the Philippine Embassy or Consulate where the divorce was obtained or by the concerned foreign diplomatic or consular mission in the Philippines. Original and CTC of Philippine Court recognition of foreign divorce decree must also be present and the PSA Marriage Contract with the annotation of the Divorce Decree.
For minor applicants, requirements for first time minor applicants are still prescribed, in addition to the passport of minor.
Aliens who acquired Filipino citizenship or applicants with foreign-sounding family names (e.g. LIM, CHO, SY, LEE, SAMADI, SINGH, etc.):
- Bureau of Immigration Identification Certificate
- Certificate of Naturalization
- Election of Filipino Citizenship / Oath of Allegiance
- If necessary,
- Marriage Certificate of parents (duly authenticated by PSA)
- PSA Birth Certificate of applicant or applicant's brother or sister
Applicants who availed of Dual Citizenship or Election of Philippine Citizenship under RA 9225:
- Identification Certificate
- Oath of Allegiance
- Order of Approval
- Report of Birth (for those born abroad)
For Muslim Applicants (same general requirements stated above)
For converts who would like to use their Muslim name:
- Annotated Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (PSA) bearing the Muslim name
- National Commission on Muslim Filipinos (NCMF) or Office on Muslim Affairs(OMA) Certificate of Conversion
If applicant is a DOST Scholar (same general requirements stated above):
- Secure a certification or clearance from DOST
