This procedure explains various ways to Delayed Birth Registration in Zamboanga Peninsula.
Apply In PersonEdit
Philippine-born applicants
- Here's how you can register a child's birth beyond the 30-day period (delayed registration). First, you will need to check that there is no existing record of the birth. You can do this by inquiring at your local civil registry (where the child was born).
- After inquiring at your local civil registry, visit any Philippine Statistics Authority office and ask them to confirm that there is no existing records in their database related to the child's birth. You will also need to secure a Negative Results Certification (NRC) from PSA. This is one of the requirements you will need to submit when you begin the late registration process.
- Next, you will need to prepare all the documents you will need to submit to your local civil registry. See list provided under 'Required Documents' section.
- Once you have all the needed documents head to your local civil registry to file your application for late registration.
- Pay the late registration fee.
- Your application will be evaluated by the civil registrar. A notice will be posted on the bulletin boards of the city or municipality for a period of not less than ten (10) days to inform the public on your pending application. If after ten (10) days no one opposes your application the civil registrar will continue processing your registration.
- The civil registrar will check if all the documents that you have submitted are authentic and correct.
- The Office will let you know if your registration has been approved and when can you collect the birth certificate.
Filipino born abroad
- Here are the procedures you will need to follow to register a child born abroad. For this, application will be processed by the Department of Foreign Affairs, Office of Consular Affairs (for applicants in the Philippines) or the Philippine Consul's Office in the country or place where the child was born (for those who are currently based abroad).
- First, you will need to prepare all the needed requirements. Please note that documentary requirements may vary depending on where the child was born and the Consular Office processing your application. Provided under 'Required Documents' section are some of the documents you will be required to submit. You may also check your local Consular Office's website in case the office requires additional documents to be submitted.
- Submit your application and all the required documents to DFA's Office of Consular Affairs or the Philippine Consul's Office.
- Pay the consular fee.
- Applications are normally processed in 2 to 4 months. Some consular offices issues a tracking number to their applicants to make it easier for them to monitor the progress of their application. In case you don't have a tracking number it might be good for you to take note of the contact details of the consular office processing your application. This way you can give them a call to follow up.
Note: The Department of Foreign Affairs, Office of Consular Affairs are processing applications on APPOINTMENT BASIS, you will need to email [email protected] to set an appointment.

Philippine-born applicantsEdit
If the person to be registered is below the age of eighteen (18):
- Four (4) copies of the Certificate of Live Birth duly accomplished and signed by the proper parties
- Accomplished Affidavit for Delayed Registration at the back of Certificate of Live Birth by the father, mother, or the guardian
- Any two of the following documentary evidences which shows the name of the child, date and place of birth, and name of mother (and name of father, if the child has been acknowledged):
- Baptismal certificate;
- School records (nursery, kindergarten, or preparatory);
- Income tax return of parent/s;
- Insurance policy;
- Medical records; and
- Others documents, such as barangay captain's certification.
- Affidavit of two disinterested persons who might have witnessed or known the birth of the child.
- Additional documents: In the delayed registration of the birth of an alien, travel documents showing the origin and nationality of the parents shall be presented in addition to the requirements mentioned.
If the person to be registered is older than eighteen (18) years old:
- Applicant will need to submit all documents mentioned above.
- Certificate of Marriage, in case the applicant is married.
- Additional documents: In the delayed registration of the birth of an alien, travel documents showing the origin and nationality of the parents shall be presented in addition to the requirements mentioned.
Filipino born abroadEdit
- Report of Birth Form
- Birth Certificate issued by the country of birth (original copy should be submitted to the DFA). Note: DFA mentioned specifics related to the Birth Certificate to be submitted. To know more please click this - LINK.
- First passport or Travel Document of the Child/Subject. Have the bio data pages photocopied. In case of unavailability, execute an Affidavit of Non-submission of Document and attach a copy valid/recent passport or any valid ID.
- Authenticated Marriage Certificate of Parents from the Philippine Statistics Authority (PSA) / Foreign Marriage Contract.
- For unmarried parents:
- If child/subject is using the surname of the father, kindly submit an Affidavit to Use the Surname of the Father (AUSF) to be executed by the mother and Affidavit of Acknowledgement of Paternity to be executed by the father. If the two (2) affidavits will be executed in the Philippines, these should be registered first as Legal Instruments at the Local Civil Registry Office of the place of execution and authenticated by the DFA. If executed abroad, these should be registered first at the Foreign Service Post (FSP) of the country of residence of the parent(s) or at the FSP nearest the place of residence.
- If child/subject is using the surname of the mother, kindly submit the Birth Certificate of the mother and an Affidavit of Illegitimacy, duly notarized.
- Passport of Parents (valid at the time of the birth of the child) plus current/valid passports of both parents:
- Photocopies of bio data pages
- In case of unavailability, execute an Affidavit of Non-submission of Document and attach a copy valid/recent passport or any valid ID [(i.e. Green card or permanent resident card, copy of visa (at the time of birth of the child), working permit if working abroad at the time of birth of the child]
- For applicants born in USA and Canada, kindly submit any of the following documents of parent/s: USA/Canada Visa, USA/Canada Working Permit and Green Card of Permanent Resident Card
- If Filipino parent/s had acquired foreign citizenship (i.e. American, Australian, British or Canadian) and re-acquired Philippine citizenship, submit certificate of naturalization of parents and re-acquisition certificates of parents and of the child. If the child is 18 years old and above at the time of parent’s re-acquisition of Philippine citizenship submit an Identification Certificate for Filipino citizen issued by the Bureau of Immigration.
- Five (5) recent passport size photos of the child/subject
- Notarized Affidavit of Delayed Registration [required only for children/subjects who are more than one (1) year old]
- Notarized Affidavit of Two (2) Disinterested Persons [required only for children/subjects who are more than one (1) year old]
- Negative Certification of Birth Record from the PSA (CRS Form No. 1)
NOTES:
- You will need to present original documents and submit five (5) photocopies of each of the documents listed above.
- Additional documents may be required by the DFA officer evaluating your application in order to determine the citizenship, identity or eligibility of the child to be registered under the Philippine law.

EligibilityEdit
- A Philippine citizen, born in the Philippines or abroad
- Your birth was not recorded with the corresponding Local Civil Registry Office (LCRO) if you were born in Philippines and the DFA Office of Consular Affairs if you were born abroad. Even births that had been previously recorded by the LCRO but the records have been destroyed or cannot be located despite diligent search and there is no existing copy with the Philippine Statistics Authority (PSA).

- Office of Consular Affairs (DFA): Consular Fee is USD 25.00 to be paid in Philippine peso.

- For births in the Zamboanga Peninsula - process your application through your local civil registry office.
- For births abroad - if you are in the Philippines, submit your application through the Department of Foreign Affairs - Office of Consular Affairs. For applicants who are currently based abroad, go to the Philippine Embassy, Consulate or Mission that has jurisdiction over the country or region where the birth occurred.
