This procedure explains various ways to Request for Certificate of Not the Same Person (NTSP) in Zamboanga City.
You are a Philippine resident, citizen or even a former Filipino/foreign national. You can attest to the fact that you are not the person listed and/or included in the derogatory database or record such as BI’s Hold Departure Order (HDO), Watch List Order (WLO) or Black List Order (BLO). You can also be requested NTSP by an immigration officer when you are in process of visa issuance, visa extension, student desk, ACR I-card or in case of a walk-in applicant. You can actively request the NTSP although you are an Out of the country applicant. Please follow the below guidelines to do it:
- Prepare the requirements:
- Applicant under visa issuance, visa extension, student desk, ACR I-card
- The duly accomplished application form Application; Photocopy of BI General Application Forms under visa issuance, visa extension, student desk, ACR I-card. Photocopy of BI Consolidated General Application Forms under visa issuance, visa extension, student desk, ACR I-card - links:
- The duly notarized Affidavit of Denial;
- The photocopy of the applicant’s passport (data page and travel stamp page);
- One applicant 2x2 ID picture;
- The photocopy of a valid ID if applied by a representative.
- A walk-in applicant in Airport Problems Excluded/ Offloaded Passenger/ DFA Derogatory
- The duly accomplished application form Application
- The Court Clearance (if within Metro Manila);
- The NBI Clearance (if outside Metro Manila);
- The old Certificate of Not the Same Person (for cases that do not have Court or NBI Clearance and the Derogatory records is not updated additional names from issuance time of the old NTSP);
- The duly notarized Affidavit of Denial;
- The photocopy of the applicant’s passport (data page and travel stamp page);
- The photocopy of a valid ID if applied by a representative.
- For Out of the Country Applicant (former Filipino/Foreign National)
- The duly accomplished application form Applicationor email the Bureau at ([email protected]) to request the application form];
- The duly notarized Special Power of Attorney by the Philippine Embassy;
- The photocopy of the applicant’s passport (data page and travel stamp page);
- Philippine National Bureau of Investigation Clearance, certifying that the applicant is not the same person who is in the Derogatory List;
- Photocopy of representative’s valid ID.
- For general applicant
- The duly accomplished application form Application
- Signed and Sealed Court Clearance duly issued by the concerned Court or Agency that issued the Order, Letter Request or Warrant of Arrest to include the person in the Bureau’s derogatory list. The Court or Agency must certify with certainty that the applicant is not the same person as that of the Accused or Respondent based on their records of the case;
- NBI Clearance for applicant with BLO and WLO issued by the Bureau of Immigration.
- Applicant under visa issuance, visa extension, student desk, ACR I-card
- Apply for Certificate of Not the Same Person (NTSP)
- Visit the BI Main Office Location,
- You or your presentative can get support about application form at the Public Information and Assistance Unit (PIAU) at BI G/F Main Office.
- Please submit the required documents for pre-screening to the Central Receiving Unit (CRU) to process this transaction and get the Order of Payment Slip (OPS).
- Please pay the applicable fees then submit the original Official Receipt with the accomplished application form and the required documents. Hence, just present claim stub on the appointed date and time of return for releasing of Original Certificate and Receipts.
- Acknowledge receipt by signing the duplicate copy of the Certification before releasing the original Certification and Official Receipt.

A customer who still incorrectly dials the old seven-digit number will hear a special announcement saying the format has been changed to eight-digit, for a period of three months after the migration (March 18, 2019 to June 17, 2019).

- The application form is written in English language. Enter legibly all the information required into the appropriate spaces on the form. If the information required is not applicable, please write “N/A”. Do not leave any item blank. The Bureau will not act on application forms that are incomplete and incorrectly accomplished. Your answers must be in English and all entries must be in CAPITAL LETTERS using English characters only. Letters like ñ, é, ü and ç are not recognized by the system. For instance, please enter names like “Muñoz” and “Sémonin” as “Munoz” and “Semonin” respectively.
- All documents required for submission must be complied with, arranged in the order as listed in the Checklist of Documentary Requirements (CDR), compiled in a legal size (8½ x 14 in.) folder and shall be submitted within the prescribed period, if applicable. Otherwise, your application shall not be accepted.
- All sworn statements or affidavits must be original notarized.
- All civil registry documents issued in the Zamboanga City (i.e. Birth Certificate, Marriage Certificate, Death Certificate, etc.) must be original and issued by the National Statistics Office (NSO) of the Philippines.
- All foreign documents must be original and authenticated by the Philippine Foreign Service Post (FSP), which has jurisdiction over the place of issuance, or by the Department of Foreign Affairs (DFA) if said document is issued by the local Embassy in the Zamboanga City with English translation if written in other foreign language.
- If the application is filed by an authorized representative, attach a photocopy of the Bureau of Immigration (BI) Accreditation Identification (ID) Certificate or an original Special Power of Attorney (SPA) for EACH applicant with a photocopy of a valid government-issued ID of attorney-in-fact.
- The original valid passport of the applicant must be presented to the concerned front line officer for identification purposes. Thereafter, it shall be returned.
