This procedure explains various ways to Replace Lost or Stolen Solo Parent ID in Zamboanga City.

Apply In-PersonEdit

  1. Here's how you can replace your lost or stolen Solo Parent ID in person. Solo Parent ID applications are processed by your local City / Municipal Social Welfare and Development Office. If you can take a look under "Office Locations & Contacts" section, a LINK was provided which you can use to search for your LGU's contact information. It might be a good idea if you could check first details regarding the processing of the procedure before going to the city or municipal office. See if your LGU is equipped to process application online and check if do you really need to go to the office in person. Some government offices made the Solo Parent ID application form available for download through their website.
  2. Download (if available) the application form. If you do not have a printer at home it's okay, you can also ask for the form at your local City / Municipal Social Welfare and Development Office.
  3. Submit the completed form together with the notarized Affidavit of Loss.
  4. The processing officer will review your application. It may take more or less 30 days for your application to be processed.
  5. Before you leave the Office, ask the officer when can you come back to collect your ID or who can you call to follow up the status of your application.

Note: Applicants may also be required to submit an endorsement from the Solo Parent President of the Barangay and other supporting documents.

The Solo Parent ID is valid for one year. It is renewable.
