Procedure

This procedure explains various ways to Renew Solo Parent ID in Zamboanga City.

Apply In-Person

1. Here's how you can renew your Solo Parent ID in person. Solo Parent ID applications are processed by your local City / Municipal Social Welfare and Development Office. If you can take a look under "Office Locations & Contacts" section, a LINK was provided which you can use to search for your Local Government Unit's contact information. It might be a good idea to check first details regarding the procedure before going to the city or municipal office. See if your LGU is equipped to process application online, as some LGUs do. Check if do you really need to go to the office in person. Some government offices made the Solo Parent ID application form available for download through their website.
2. Download (if available) the application form. If you do not have a printer at home it's okay, you can also ask for the form at your local City / Municipal Social Welfare and Development Office.
3. Submit the completed form together with all the required documents.
4. The processing officer will review your application. It may take more or less 30 days for your application to be processed. Interviews and home visit will be conducted by the processing officer.
5. Before you leave the Office, ask the officer when can you come back to collect your ID or who can you call to follow up the status of your application.

Eligibility

Solo Parent IDs are issued to the following:

• A woman who gives birth as a result of rape and other crimes against chastity even without a final conviction of the offender, provided that mother keeps and raises the child.
• Parent left solo or alone with the responsibility of parenthood due to the following circumstances:
  • Due to death of spouse.
  • Spouse is detained or is serving sentence for a criminal conviction for at least one (1) year.
  • Physical and/or mental incapacity of spouse as certified by a public medical practitioner.
  • Legal separation or de facto separation from spouse for at least one (1) year, as long as he/she is entrusted with the custody of the children.
  • Declaration of nullity or annulment of marriage as decreed by a court or by a church as long as he/she is entrusted with the custody of the children.
• Unmarried mother/father who has preferred to keep and rear her/his child/children instead of having others care for them or give them up to a welfare institution.
• Any other person who solely provides parental care and support to a child or children.
• Any family member who assumes the responsibility of head of family as a result of the death, abandonment, disappearance or prolonged absence of the parents or solo parent.

Fees

FREE. No fees are collected.

Validity

• The Solo Parent ID is valid for one year. It is renewable.
