This procedure explains various ways to Renew Person With Disability (PWD) ID in Zamboanga City.
Apply In PersonEdit
- Here's how you can renew your Person With Disability (PWD) ID in person. First, download the application form. You can use the link provided under "Documents to Use" section. Please note that the form is from the Department of Health, it is the same form which can be found on their Philippine Registry for Persons with Disability Website. Now, some LGUs use their own form in processing PWD registration and ID request. It can be a little bit confusing which form you should use, but to be safe it is better to use the form provided by your local government unit. The forms are usually available on the LGU's website. To help you search for your LGU's contact details and website use the link under "Office Locations & Contacts" section. If you can't find your LGU's form online you can use the DOH one. And, in case you don't have a printer at home don't worry you can ask for a copy of the form when you visit your local Persons With Disability Affairs Office.
- Once you have completed the form submit it together with the original and photocopy of your expired ID.
- You will also be asked to submit a 2×2 ID picture (some LGUs might ask applicants to submit 1x1 ID picture). Might be wise to have both ID picture sizes ready when you apply.
- Submit all the required documents and pay the renewal fee (if there is any).
- Your application will be reviewed by the processing officer.
- Once your application is approved you will be directed to the PWD ID Issuance Section.

- Original and photocopy of expired ID
- New 2×2 ID picture (some LGUs might ask applicants to submit 1x1 ID picture)
- Authorization Letter (in absence of the PWD applicant)
- If you moved to a different address, you may also be asked to submit a Barangay Clearance
Note: Applicants may also be required to submit additional requirements depending on the processing officer like latest Medical Certificate or Abstract (for non-apparent disability).

ID cards are issued to any bonafide Person With Disability with permanent disabilities.
This includes persons suffering from disabling diseases resulting to the persons limitations to do day to day activities as normally as possible such as but not limited to those undergoing dialysis, heart disorders, severe cancer cases and such other similar cases resulting to temporary or permanent disability.
