This procedure explains various ways to Register a Birth in Zamboanga City.

Please follow the below guide to register a Birth:

The child was born at home

  1. Prepare the requirements:
    If the parents are married:
    a. On-time registration
    • The Marriage Contract of parents;
    • Signature of attendant at birth.
    b. Delayed registration
    • NSO Negative Certification of Birth (1945-2005);
    • Baptismal Certificate of the Child;
    • Marriage Contract of Parents;
    • Signature of attendant at birth (if still living);
    • Affidavit of two disinterested persons;
    • Current Community Tax Certificate of the informant;
    • At least three documentary evidences/ public documents showing correct full name, date of birth and place of birth of the child.
    If the parents are not married:
    a. On-time registration
    • Signature of attendant at birth;
    • Signature of the father (if the child is to be acknowledged or the father is willing to admit paternity of the child);
    • Current Community Tax Certificate of the father.
    b. Delayed registration
    • NSO Negative Certificate of Birth (1945-2005);
    • Baptismal Certificate of the child;
    • Signature of the father & his current Community Tax Certificate (if the child is to be acknowledged or if the father is willing to admit paternity of the child);
    • Sworn statement of the mother/ or sworn statement of the father/grandmother/grandfather in lieu of the mother (if the mother is deceased or her whereabouts are unknown) and current Community Tax Certificate;
    • Duly Notarized Affidavit of two disinterested persons;
    • At least three documentary evidences showing correct full name, place of birth and date of birth of the child.
  2. Register the Birth.
Please visit a suitable Municipal Civil Registry Office (MCR) Municipal Civil registry Office and present the required documents. Follow the entire officer’s guide to complete the form no.102 Certificate of Live Birth or COLB Certificate of live birth. If you are in case of delayed registration, please pay the corresponding fees at the Municipal Treasurer’s Office and receive your Official Receipt. Submit the Official Receipt to the Registration Officer/Clerk (if applicable) and waiting for he/her release and records the COLB. In case of delayed registration, you will be inform about the date of release of the COLB in compliance with the 10 days posting period and just come back and claim the duly registered certificate of Live Birth on the appointed date. Normal case, you just claim the registered documents when the officer completes encoding and assigning registry number to the COLB.

The child was born in the hospital/maternity clinic

  1. Prepare the requirements
    If the parents are married:
    a. On-time registration
    • Duly accomplished quadruplicate copies of Certificate of Live Birth with the signature of attendant at birth, the informant and the hospital staff who prepared the Certificate of Live Birth.
    b. Delayed registration
    • Duly accomplished quadruplicate copies of Certificate of Live Birth with the signature of attendant at birth, the informant and the hospital staff who prepared the Certificate of Live Birth;
    • Current Community Tax Certificate of the informant.
    If the parents are not married:
    a. On-time registration
    • Duly accomplished quadruplicate copies of Certificate of Live Birth with the signature of attendant at birth, the informant and the hospital staff who prepared the Certificate of Live Birth;
    • Signature of the father (if the child is to be acknowledged or if the father is willing to admit paternity of the child) and his current Community Tax Certificate.
    b. Delayed registration
    • Duly accomplished quadruplicate copies of Certificate of Live Birth with the signature of attendant at birth, the informant and the hospital staff who prepared the Certificate of Live Birth;
    • Signature of the father and his current Community Tax Certificate (if the child is to be acknowledged or if the father is willing to admit paternity of the child);
    • Duly notarized sworn statement of the mother/father/guardian and current Community Tax Certificate;
    • Duly Notarized Affidavit of two disinterested persons.
  2. Register the Birth:
Please visit a suitable Municipal Civil Registry Office (MCR) Municipal Civil Registry Office and present the required documents. The officer will review the documents for completeness and receive the documents. In case of delayed registration, you must follow the entire officer’s guide to be issued order of payment form, instruction to pay fees and receive the Official Receipt from the Revenue Collection Officer/Clerk. Return to the Registration Officer/Clerk and submit the Official Receipt to be informed about the date of releasing the COLB in compliance with the 10 days posting period and you should come back to claim the duly registered certificate of Live Birth on the appointed date. For normal case, you just claim the duly registered certificate of Live Birth after the Registration Officer/Clerk assigned registry number to the COLB.

Out-of-town registration of Births:
This is one kind of delayed registration for those who were born outside the municipality or far provinces to married/unmarried parents that processing by relatives or the person himself.

  1. Prepare the requirements:
    • Negative result from NSO;
    • Baptismal Certificate;
    • Marriage Contract (if married);
    • Voter’s Registration Record;
    • Duly Notarized Affidavit of two disinterested persons;
    • Duly Notarized Affidavit for delayed registration;
    • Current Community Tax Certificate;
    • Other supporting documents that show correct full name, place of birth and date of birth of the child.
  2. Register the Birth:
Please visit a suitable Municipal Civil Registry Office (MCR) Locations and present the required documents. Follow the entire officer’s guide to provide the necessary information during the interview and wait for the registration officer/Clerk prepare the Certificate of Live Birth. Please review for correctness and completeness, affix signature on the space provided, and pay Postal Money Order (PMO) at the Post Office from Postmaster. Return to the Registration Officer/Clerk and submit the PMO then wait for the instruction from the officer. Now you are in transmitted document process, normally you must wait for result from MCR after 2-4 weeks for the Issuance of the registered out-of-town COLB.

You are parent/ guardian/ attendant at birth/hospital and clinic authorities, and persons who have reached legal age but whose facts of births have not been reported at the Civil Registrar’s Office, you can register the Birth in Philippines. Normally, the birth of the child should be registered at the office of the Civil Registrar within 30 days from the time of birth if you do not want to join in delayed registration.

Need for the DocumentEdit

  • Birth registration is a permanent official record of a child. The child who is not registered at birth is in danger of being denied the right to an official identity, a recognised name and a nationality.

What are the exceptional cases for the place of registration of birth?

  • When a child is born aboard a vehicle, vessel or airplane while in transit withing Philippine territory and the exact place of birth could not be ascertained, the birth shall be recorded in the civil register of the city or municipality of the mother destination or where the mother habitually resides;
  • When the child is born aboard a vessel or airplane en route to the Philippines, the birth shall be recorded in the civil register of the city or municipality where the mother habitually resides, if she is a resident of the Zamboanga City and if either the father or the mother or both parents are citizens of the Philippines. When the parents are both foreigners but not residents of the Philippines, the birth may be recorded in the civil register of Manila, if they so desire;
  • For delayed registration of events where the city/municipality has been divided or integrated, the registration of birth shall be made in the mother city/municipality, which has jurisdiction over the reported place of occurrence of birth.

  • The birth certificate is used to authenticate one's identity and nationality, and assist with obtaining government-issued identity documents, such as a passport or driver's license.
