ProcedureEdit

Steps for Renewal

  1. Secure and fill out an application form and attach required documents.
  2. Submit application with complete requirements.
  3. Check the database for any derogatory record.
  4. Get an Order of Payment Slip (OPS).
  5. Pay the required fees.
  6. Submit copy of Official Receipt.
  7. After evaluation application and approved, the ACR I-Card shall be printed.
  8. Present claim stub and identification (ID) card.

Required DocumentsEdit

For Student Visa

  • Duly accomplished renewal/reissuance application form;
  • The original ACR I-Card;
  • Photocopy of passport (biodata page, visa implementation page and visa page with latest arrival stamp);
  • Photocopy of the Student Desk-approved Order granting 9(f) visa extension; and
  • Photocopy of Official Receipt showing payment of the applicable fees.

For Re-Registration

  • Duly accomplished renewal/reissuance application form;
  • The original ACR I-Card;
  • For Immigrant: Photocopy of issued Immigrant Certificate of Residence (ICR);
  • For Non-Immigrant: Certified True Copy of Board of Commissioner's (BOC) Order;
  • Photocopy of parent's documents (ACR I-Card/Passport);
  • Photocopy of Passport (biodata page, visa implementation page and visa page with latest arrival stamp);
  • Two (2) pieces "2 x 2" color photos (in white background; without eyeglasses; taken within the last six (6) months); and
  • Photocopy of Official Receipt showing payment of the applicable fees.

For Change of Visa Status

  • Duly accomplished renewal/re-issuance application form;
  • The original ACR I-Card;
  • Photocopy of passport (biodata page, visa implementation page and visa page with latest arrival stamp);
  • Photocopy of the Board of Commissioner's (BOC) Order; and
  • Photocopy of Official Receipt showing payment of the applicable fees.

For Change of Visa Status

  • Duly accomplished renewal/re-issuance application form;
  • The original ACR I-Card;
  • Photocopy of Official Receipt showing payment of the applicable fees.

For Re-Issuance of ACR I-CardEdit

For Lost ACR I-Card

  • Duly accomplished renewal / re-issuance application form;
  • Letter-request for reissuance of the ACR I-Card;
  • Duly notarized Affidavit of Loss;
  • Police Report on ACR I-Card loss;
  • Photocopy of the Passport (biodata page, visa implementation page and visa page with latest arrival stamp); and
  • Photocopy of Official Receipt showing payment of the applicable fees.

For Damaged ACR I-Card

  • Duly accomplished renewal/re-issuance application form;
  • Original ACR I-Card; and
  • Photocopy of Official Receipt showing payment of the applicable fees.

For Rejected ACR I-Card

  • Duly accomplished renewal/re-issuance application form;
  • Original ACR I-Card; and
  • Photocopy of Official Receipt showing payment of the applicable fees.

Any person with a previous ACR I-Card.



FeesEdit

RE-ISSUANCE - US$20 ( Peso equivalent )

  • Lost card
  • Damaged card
  • Rejected card( wrong entries or information printed in the card due to fault or negligence of the applicant )
  • Amendments

RENEWAL - US$50 ( Peso equivalent )

  • Expiration of one (1) year validity of the card
  • Re-registration upon reaching the age of 14
  • Change of Visa Status
  • ACR Replacement (Alien Registration Act of 1950, as amended)
