This procedure explains various ways to Obtain a Duplicate ACR Identity Card (I-Card) in Zamboanga City.


You are a foreign national. You hold a valid ACR I-card but in case of lost card, damaged card, amendments (address, name, nationality, civil status, etc.), or rejected card (wrong entries or information printed in the card due to fault or negligence of the applicant). You can apply for a Duplicate ACR Identity Card (I-Card) by following these procedures: You are a foreign national. You can apply for a Duplicate ACR Identity Card (I-Card) by following:

  1. Prepare the requirements:
    1. The accomplished ACR I-Card renewal/re-issuance application form link;
    2. The original ACR I-Card (if applicable);
    3. The photocopy of passport bio-page, visa implementation page and latest admission with valid authorized stay;
    4. The Certified true copy issued by the Records Section of the Board of Commissioner (BOC)’s Order granting the visa (for internal verification purposes); and
    5. The Certified true copy issued by the Records Section of the Alien Certificate of Registration (ACR)/Immigrant Certificate of Residence (for paper-based ACR holders).
    6. The Additional documents:
      • For a lost ACR I-Card: The letter request for re-issuance of the ACR I-Card; The Affidavit of Loss; and The Police Report.
      • For Amendment: Name (The Court Order (if applicable), the Amended/Corrected Birth Certificate, and the Affidavit of Amendment of Name/Inclusion of Name); Citizenship/Nationality (The Official document proving change of citizenship/nationality, the Affidavit of Change of Citizenship/Nationality); Civil Status (The Official document proving change of civil status, the Affidavit of Change of Civil Status); Address (The Certified of Residence from the Barangay Captain, the Photocopy of ACR I-Card, and the Affidavit of Change of Address).
      • For a Damaged ACR I-Card: The Original ACR I-Card (to be surrendered); the Official receipt(s) of payment for applicable fees; and the Photocopy of passport bio-page and passport pages bearing the visa implementation (non-immigrant) and/or latest arrival stamp (tourist).

  2. Apply for a Duplicate ACR Identity Card (I-Card)
    Visit the BI Main Office or other Immigration Offices link that have the Alien Registration Program (ARP) service. Submit the completed application form and the required documents and the office will check the database for any derogatory record from you. Follow officer’s guiding to take an Order of Payment Slip (OPS), pay the applicable fees and submit copy of the Official Receipt. After evaluation application and approved, the ACR I-Card shall be printed. Present claim stub and identification (ID) card to obtain the duplicate ACR I-Card.

A customer who still incorrectly dials the old seven-digit number will hear a special announcement saying the format has been changed to eight-digit, for a period of three months after the migration (March 18, 2019 to June 17, 2019).

Need for the DocumentEdit

If your ACR Identity Card has been lost, stolen or damaged, it is a must to obtain a duplicate for future use. An ACR I-Card is only valid for five years, so it must be renewed.

  • The application form is written in English language. Enter legibly all the information required into the appropriate spaces on the form. If the information required is not applicable, please write “N/A”. Do not leave any item blank. The Bureau will not act on application forms that are incomplete and incorrectly accomplished. Your answers must be in English and all entries must be in CAPITAL LETTERS using English characters only. Letters like ñ, é, ü and ç are not recognized by the system. For instance, please enter names like “Muñoz” and “Sémonin” as “Munoz” and “Semonin” respectively.
  • All documents required for submission must be complied with, arranged in the order as listed in the Checklist of Documentary Requirements (CDR), compiled in a legal size (8½ x 14 in.) folder and shall be submitted within the prescribed period, if applicable. Otherwise, your application shall not be accepted.
  • All sworn statements or affidavits must be original notarized.
  • All civil registry documents issued in the Zamboanga City (i.e. Birth Certificate, Marriage Certificate, Death Certificate, etc.) must be original and issued by the National Statistics Office (NSO) of the Philippines.
  • All foreign documents must be original and authenticated by the Philippine Foreign Service Post (FSP), which has jurisdiction over the place of issuance, or by the Department of Foreign Affairs (DFA) if said document is issued by the local Embassy in the Zamboanga City with English translation if written in other foreign language.
  • If the application is filed by an authorized representative, attach a photocopy of the Bureau of Immigration (BI) Accreditation Identification (ID) Certificate or an original Special Power of Attorney (SPA) for EACH applicant with a photocopy of a valid government-issued ID of attorney-in-fact.
  • The original valid passport of the applicant must be presented to the concerned frontline officer for identification purposes. Thereafter, it shall be returned.
