Zamboanga City - Modify or Change Information in ACR Identity Card

This procedure explains various ways to Modify or Change Information in ACR Identity Card in Zamboanga City.
You are a foreign national under immigrant and non-immigrant visa including holder of Temporary Visitor’s Visa, who have stayed for more than fifty-nine days in the Philippines. You hold an ACR Identity Card (ACR I-Card) but some entries as address, name, nationality, civil status, etc. need to be amened. You can apply for Modify or Change Information in ACR I-Card by following:
- Prepare the requirements Form:
- The Court Order (if applicable), the Amended/Corrected Birth Certificate, and the Affidavit of Amendment of Name/Inclusion of Name (for case of Name amendment);
- The Official document proving change of citizenship/nationality, the Affidavit of Change of Citizenship/Nationality (for case of Citizenship/Nationality amendment);
- The Official document proving change of civil status, the Affidavit of Change of Civil Status (for case of Civil Status amendment);
- The Certificate of Residence frum the Barangay Captain, the Photocopy of ACR I-Card, and the Affidavit of Change of Address (for case of Address amendment);
- Submission of additional supporting documents may be required by the Bureau for further verification and evaluation of application;
- The application form Application

- Apply for Modify or Change Information in ACR I-Card
Visit the BI Main Office or other Immigration Offices Office Locations that have the Alien Registration Program (ARP). Submit the completed application form and the required documents and the office will check the database for any derogatory record from you. Follow officer’s guding to take an Order of Payment Slip (OPS), pay the applicable fees and submit copy of the Official Receipt. After evaluation application and approved, the ACR I-Card shall be printed. Present claim stub and identification (ID) card to obtain the revised ACR I-Card.

Note: NTC issued Memorandum Order No. 10-10-2017 last October 27, 2017, directing all telecommunications providers in the country to migrate all its customers to eight-digit telephone numbers.

A customer who still incorrectly dials the old seven-digit number will hear a special announcement saying the format has been changed to eight-digit, for a period of three months after the migration (March 18, 2019 to June 17, 2019).
