Procedure

This procedure explains various ways to Apply for Person With Disability (PWD) ID Card in Zamboanga City.

1. To obtain a PWD Card, applicant may apply for the card with the assistance his/her caregiver by submitting an application to the City or Municipal Mayor, or Barangay Captain's office.
2. A completed application must be submitted together with these following documents:
   1. Two 1x1 recent ID pictures with the names, and signatures or thumb marks at the back of the picture.
   2. One (1) Valid ID
   3. Document to confirm the medical or disability condition.
3. The form may be accomplished manually or you may also complete your application using the Online Registration System:
   1. Log in to the DOH-PPWDRS and access the online registration. - link
   2. Enter accurately and completely the required PWD registration data.
   3. Print the accomplished form.
   4. Affix one (1) ID picture on the accomplished form, and staple the other ID picture with the PWD-RF.
   5. Attach copy of the document to confirm the medical or disability condition with the PWD-RF.
4. The office will process your application, issue the ID Card and submit the accomplished PWD-RF and attached requirements and/or documents to the City or Municipal Social Welfare Development Office for data encoding into the DOH-PPWDRS.

• Completed application
• Two 1x1 recent ID pictures with the names, and signatures or thumb marks at the back of the picture.
• One (1) Valid ID
• Document to confirm the medical or disability condition. Document must be issued by the following:
  • Apparent Disability Medical Certificate - Licensed Private or Government Physician
  • School Assessment - Licensed Teacher duly signed by the School Principal
  • Certificate of Disability - Head of the Business Establishment of the head of Non-Government Organization
• Non-Apparent Disability Medical Certificate - Licensed Private or Government Physician

Eligibility

ID cards are issued to any bonafide Person With Disability with permanent disabilities.

This includes persons suffering from disabling diseases resulting to the persons limitations to do day to day activities as normally as possible such as but not limited to those undergoing dialysis, heart disorders, severe cancer cases and such other similar cases resulting to temporary or permanent disability.

Fees

The initial PIC is free of charge.

Renewal due to expiration, loss or damage shall be charged a minimal fee for the new PIC.

Validity

Person With Disability (PWD) ID is valid for 3 years.
