Procedure

Apply In-Person:

1. To apply for a change of name, in Zambia, the applicant will have to contact the registrar office of your area, for the main office visit link.
2. Application form for change of the name will be available in the office.
3. Make sure that you have the documents that are listed under the “Required Documents” section of this page.
4. Go to the registrar with a statutory declaration made before a Commissioner for Oaths, stating that the applicant absolutely renounces the use of his former name or names and assumes a new name or names.
5. Complete the form provided by the registrar with all information as the registrar may require to complete the application form.
6. The registrar will forward the completed document, together with the statutory declaration and the national registration card, to the Chief Registrar.
7. The Chief Registrar will amend the register in a manner he may deem fit.
8. The Chief Registrar will cancel the National Registration Card forwarded to him by the registrar.
9. The registered applicant will be issued a new National Registration Card upon payment of the appropriate fee.
10. Collect the card with the new name from the office.
