Apply In-Person:

  1. To appeal for a Denial or Revocation of Identification Card in Zambia, the applicant will have to visit the agency for persons with disability at the district or provincial office, for the main office visit link.
  2. Make sure to have all the documents that are listed under the “Required Documents” section of this page.
  3. If the applicant is denied an identification card or revoked, then the aggrieved person has the right to file an appeal and have a hearing.
  4. An application form will be provided to the applicant upon visiting the agency.
  5. Once understanding the guideline properly, the applicant has to fill-in all the necessary information in the application form.
  6. Submit the application form with necessary attachments to the office.
  7. Pay the fee as given in the “Fees” section of this page.
  8. The Director General shall hear the case or appoint an officer to hear the case.
  9. The complainant is entitled to be represented by parents, family members, friends, lawyers and he/she is allowed to present witnesses and other evidence.
  10. The aggrieved persons shall submit all the evidence relied to the Director General.
  11. Following the hearing, the Agency will issue a written decision.
  12. When Decision is in the applicant’s favour, they will receive the card in a stipulated amount of time.
  13. If the aggrieved person disagrees with the decision made at the hearing, he/she may file a complaint in the Magistrate Court of the county where they live.

  • A person whose request for the Disability Identification Card has been denied can appeal for the revocation.

  • Application Fee: Pay as per the authority’s request.

  • This complaint must be filed no later than 35 days after the date that the hearing decision was issued. A magistrate will then review the decision.
